چکیده

Congenital tuberculosis (CTB) is a rare disease with a high mortality rate, requiring high index of suspicion to diagnose because of non-specific presentation. This report describes a case of 6 month male child with congenital tuberculosis with possible placental transmission. Infant had fever, fast breathing and cough since 20 days of life. Ultrasonography (USG) abdomen showed granulomatous lesions in liver. Although mother was diagnosed to be having tuberculosis during 3rd week after delivery and started on anti-tuberculosis treatment (ATT), child remained undiagnosed for 6 months and didn’t get appropriate treatment. Fever, respiratory distress and cough initially improved after starting ATT but patient worsened after 2 weeks of ATT which could not be attributed to cause other than tuberculosis paradoxical reaction (TB-PR). Patient was given steroid following which he gradually improved. This case highlights the importance of keeping high index of suspicion for congenital TB in infants, especially in developing nations, and need for early diagnosis and treatment for the survival of theses infants. PR should be considered even in infancy if patient shows worsening of symptoms after initial improvement that couldn’t be explained otherwise. Prolonged paradoxical reaction to anti-tuberculous treatment after discontinuation of TNF-alpha- blocker therapy with adalimumab. Rare clinical documentation

In the past decades, tumor necrosis factor alpha (TNF-a) antagonist has been a milestone in the treatment of many chronic inflammatory diseases. TNF antagonist can increase patients' susceptibility to many different kinds of infections especially those requiring granuloma formations despite regular performance of Screening for latent tuberculosis infection (LTBI). We report 2 cases of patients ...

Paradoxical upgrading reaction in extra-pulmonary tuberculosis: association with vitamin D therapy

SETTING Glasgow, Scotland, UK. BACKGROUND Paradoxical reactions in tuberculosis (TB) are a notable example of our incomplete understanding of host-pathogen interactions during anti-tuberculosis treatment. OBJECTIVES To determine risk factors for a TB paradoxical reaction, and specifically to assess for an independent association with vitamin D use. DESIGN Consecutive human immunodeficienc...

Polymerase Chain Reaction for the Diagnosis of Tuberculosis

Dear Editor-in-Chief We read with interest the study by Khazaei et al. (1) in which the authors have nicely concluded that PCR is more sensitive test than Ziehl-Neelsen staining and histo-pathological examination for the diagnosis of tuberculosis (TB). They have rightly pointed to use PCR, selectively, in acidfast bacilli negative paucibacillary forms of TB. However, we intend to highlight few...

Paradoxical response as a cervical lymph node enlargement after termination of anti-tuberculosis treatment in a patient with pulmonary tuberculosis

A paradoxical response is not uncommon in non-HIV-infected patients, particularly those with extra-pulmonary tuberculosis. It is defined as the radiological and clinical worsening of a previous lesion or the development of new lesion during anti-tuberculosis therapy. The paradoxical response has been attributed to host immunologic reactions, such as a delayed hypersensitivity or a response to m...
